Abstract: Electronic Health Records (EHR) System evolved from the manual records used to store patient’s data and document the activities of the patient in the hospital. In the previous system, data redunancy and loss of data files were the major problems faced along with the inability to easily book an appointment with the doctor. Therefore, the of this project is to automate the process of recording health records in medical institutions and to manage patient data properly. In order to achieve its aim, a database for storing patients’ information was created and an online interface for recording and display of information was designed. The developed web based electronic health record system was then implemented. The EHR developed was able to store patient information, as well as handle online customer registration and booking and cancellation. It also allowed the administrator to modify the records of patients’ and staff information that are within the institution. The EHR is very useful and is a good upgrade from the previous system. It is recommended that the system is improved upon to increase the scope and productivity of the system.
